Please accept this letter as a formal request and authorization for the above referenced Distribution Company (TDU) to release energy usage data, including kWh, kVA or KW, and interval data (if applicable) at the following location(s) to  O & M Resources.  This information request shall be limited to no more than the most recent 12-month period of service.  If the ESI ID(s) are IDR accounts, please indicate whether summary level and/or interval data is required.

I further authorize and agree that O & M Resources, Inc. shall solely and exclusively represent us in obtaining usage, billing and credit information, energy supply quotes, evaluating offers, copies of executed contracts and assisting in contract negotiations and execution with all Retail Electric Providers (REP).  Upon execution of an agreement with a REP of our choice, O & M Resources will receive a monthly commission for these services from such REP. REP’s are requested to forward all offers, communications and correspondence to O & M Resources. This authorization and agreement is effective from the date indicated herein for a minimum of thirty days, after which either party shall have the right to terminate this agreement with thirty days advanced written notice.  This agreement will continue concurrently with the term of any electricity service agreement between our company and the REP of our choice.
